Procedures for Your First Exam Order
To purchase the Certified Food Safety Manager™
Exam, all trainers must complete each of the following steps before ordering
exams for the first time:
- Read the Test Administrator Manual.
- Complete, sign and notarize the Test
Administrator Agreement form.
- Complete both pages of the Reference form by documenting three references. At least two references
must be from individuals who have known you for more than one year.
Please be sure to indicate your status as a Registered Trainer of
NEHA Food Safety Training. All reference forms downloaded from NEHA Food Safety Training's
Website automatically include the Registered Trainer logo for this
purpose.
- Submit both forms
above to the following address, not to NEHA Food Safety Training: National Registry of Food
Safety Professionals,
5728 Major Blvd., Ste 750,
Orlando, FL 32819.
- To
expedite the processing of your paperwork, you may fax both forms
to the National Registry at (407) 352-3603 in addition to mailing
the originals to the address above.
- Once both forms
have been processed by the National Registry, you will be notified
of your test administrator and trainer numbers, which you will need
to provide to NEHA Food Safety Training each time you order exams.
Important Notes:
- Exams are the same price when purchased
through NEHA Food Safety Training as they would be if purchased directly from
the National Registry, however, registered trainers receive a discount
on the price of books purchased from NEHA Food Safety Training. NEHA Food Safety Training
offers the National Registry's exams as a convenience to trainers
who prefer a single point of purchase for books and exams.
- When exams are purchased through NEHA
Training, NEHA Food Safety Training issues the invoice after the test
is administered. Trainers are charged only for the number of exams
actually used, as long as all unused exams are returned to the National
Registry.
